How To Transmogrify:
Equip an item that is suitable for transmogrification.
Have an item in your bags that is suitable to be used to transmogrify the equipped item
Talk to Transmogrifier and select the item slot. Then select the item you want to transmogrify to.
Click to proceed when prompted and your items are transmogrified.

Any of the limitations should be easy to modify if you know how, just taking the quality check out can be quite fun
(Transmogrify any quality item to any quality, so you can have a crap gear with epic stats or epic gear with crap stats :3
Also brings artifacts etc up for transmogrification)

Maybe my question is stupid but, where do we find the NPC transmogrify?
Do I associate the script with any NPC?

(I'm noob with C++...)

Yes.
You should create your own NPC and stick NPC_Transmogrify to it's scriptname field in creature_template.
You should also have the script added to your core.

Just a simple problem, the transmog doesn't stay after a reconnexion :x..

It doesn't update an integer in the database, therefore it probably won't stick after you log out or exit the game.
